COVID Vaccination Policy

As of December 8, 2023, if you work at a Lab site, you must acknowledge you are up-to-date with your COVID vaccination or decline to receive it by completing an attestation. New hires must complete the attestation within 14 days of their start date. Proof of vaccination is not required. 

Represented employees should consult their collective bargaining agreement for information on COVID vaccination. 

Those who work remotely are encouraged to get a COVID vaccine through their healthcare provider or a pharmacy.

To complete the attestation, visit clinic.lbl.gov>Message Center>COVID Vaccination Declaration  Questionnaire 2023-24. Select the proper declaration before clicking on submit.
